The Anatomical Repair Technique to Correct Penile Hypospadias Provides Excellent Long-term Function with Low Complications

Introduction There are various techniques described for repair of penile hypospadias. Objective To present the anatomical technique and to discuss lessons learned from this over 18 years. Methods All consecutive patients with hypospadias who underwent urethra, spongiosum, bucks dartos fascia foreskin by 3rd author, a minimum 1 year follow up January 2003 March 2020 at Royal Manchester Children's Hospital, United Kingdom, were included in study. No formal ethics approval taken as study was done service evaluation. Perioperative data recorded prospectively departmental database. SIPP 21 used statistical analysis. Fisher’s Exact Test sub-group Findings considered statistically significant p<0.05. Results 368 had surgery median age months. 94.6% no complications, 4.1% minor 1.3% complications. 77.5% distal ectopic meatus. The mean length urethroplasty 8mm. widen urethral plate, meatoplasty (M) performed 34% plate incision (UPI) 14%. Catheters 71% duration 7 days, 40% 6F catheter. Regards meatal stenosis only causative factor UPI 6fr P value=0.02. Conclusions Anatomical provides completely normal penis minimal Meatoplasty widens urethral-plate reduces risk stenosis.
